When No One Believed Him: A 4-Day Functional Evaluation That Changed Everything

After 7 years of debilitating dizziness, falls, fatigue, and a diagnosis that didn’t seem to fit, a client was referred for what was intended to be a two-day Functional Capacity Screen Work Evaluation (FCSWE).
What unfolded turned into a 4-day evaluation, due to the complexity of his case:
Persistent dizziness
Undiagnosed cognitive dysfunction
Visual blurriness and field neglect
PTSD and severe anxiety
A long-standing misdiagnosis of psychiatric origin
Despite appearing socially appropriate, our Functional Capacity Screen Work
Evaluation (FCSWE) revealed the depth and extent of his impairments. Prior
assessments had missed the underlying causes and his symptoms were often
misunderstood or dismissed as psychological overlay.
Key Findings from the FCSWE:
Severe vestibular dysfunction with high risk for falls
Significant cognitive dysfunction affecting planning, memory, and daily functioning
Visual field neglect and blurred vision, impacting daily function and safety
High levels of fatigue and post-concussion symptoms exacerbated by reading and screen use (television, phone).
These findings resulted in immediate recommendations for:
24/7 attendant care support
Discontinuation of driving
Vision therapy and neuro-optometry referral
Ongoing emotional and psychological support
Therapies to maintain his physical functioning
The client’s wife, who had been his sole caregiver for seven years—and had suffered a
heart attack early on in his recovery—shared her response:
